Usual Pathogens

Trichophyton spp
Epidermophyton spp
Microsporum spp

Topical therapy with one of:

Empiric Therapy Dose Duration
Ciclopirox 1% Apply bid 2-4 weeks
or    
Clotrimazole 1% Apply bid 2-4 weeks
or    
Ketoconazole 2% Apply daily 2-4 weeks
or    
Miconazole 2% Apply bid 2-4 weeks
or    
Terbinafine 1% Apply daily - bid 2-4 weeks
or    
Tolnaftate 1% Apply bid 2-4 weeks
