Therapy	Dose	Duration
Amphotericin B, liposomal or lipid-complex	5 mg/kg IV daily
+
Flucytosine	25-37.5mg/kg PO q6h

Therapy	Dose	Duration
Fluconazole	400-800mg IV/PO daily	Continue until all clinical and radiological signs have resolved and medical devices removed.
or
Voriconazole	6 mg/kg IV q12h x 2 doses, then 4mg/kg IV q12h or 400mg PO q12h x 2 doses, then 200mg PO q12h	Continue until all clinical and radiological signs have resolved and medical devices removed.

6 mg/kg IV q12h x 2 doses,

then 4mg/kg IV q12h or

400mg PO q12h x 2 doses,

then 200mg PO q12h
