Recommended Gentamicin/Tobramycin/Amikacin Dose

For amikacin, use 15mg/kg. Round dose to nearest 50mg.

Pediatric patients

• 1 month - < 9 years of age: 7-9 mg/kg based on actual body weight [ABW] 
• ≥ 9 years of age: 7mg/kg based on ABW
• Cystic fibrosis: 10mg/kg based on ABW
• Round dose to nearest 5mg
• Administer over 60 minutes

Adult patients

• 5-7 mg/kg based on ideal body weight (IBW). Use 7mg/kg for serious infections.
• Round dose to nearest 20mg
• Administer over 60 minutes

Ideal body weight (IBW)

IBW (females) = 45.5kg + [(2.3 x inches > 5 feet) or (0.92 x cm >150cm)]

IBW (males) = 50kg + [(2.3 x inches > 5 feet) or (0.92 x cm >150cm)]
