Mild:  presence of ≥ two manifestations of local infection (purulence, erythema, tenderness, warmth, or induration), cellulitis/erythema extends ≤ 2cm around the ulcer, and infection is limited to the skin or superficial subcutaneous tissues; no other local complications and no systemic signs (as described under Severe).  Exclude other causes of an inflammatory response of the skin (e.g., trauma, gout, acute Charcot neuro-osteoarthropathy, fracture, thrombosis, venous stasis).

Moderate:  local infection as defined under Mild in a patient with no systemic signs (as described under Severe) but which has ≥ one of the following characteristics: cellulitis/erythema extending >2cm, lymphangitic streaking, spread beneath the superficial fascia, deep tissue abscess, gangrene, involvement of muscle, tendon, joint or bone.
